2. OBTAIN A RETURN AUTHORIZATION NUMBER: SEND THE COMPLETED RA FORM TO THE ABOVE E-MAIL ADDRESS. ONCE THE FORM HAS BEEN RECEIVED, YOU WILL BE PROVIDED WITH YOUR RA NUMBER AND THE RETURN ADDRESS.
